failsafes should address every problem...
It also makes you realize how complex the systems are and how much we don't understand.
It's funny you ask today. I was researching the web about this topic yesterday after a conversation about Toyota throttle problems with a friend in the morning. I also thought about asking the guys here what they think the real problem is.
