Verify the disks are not scratched at all, if so - swap them.
Verify that the cartridge does not have a bent divider or that the disks are mal-seated.
Take a flashlight and peer into the drive bay. See if any crap got in there. Its possible the drive is just failing. Also, check all the cables connecting the drive.

Failing all that:
Disconnect the battery (Negative first/only),
Disconnect the cable(s) going to the drive, reconnect, rinse, lather, repeat.
Plug the drive back in.
Connect the battery.

If all that fails Check the Navigatioin A/V forum.
The drive may have just crapped out - buy a replacement on eBay. Remember to disconnect the battery before touching its cables.
